Facial sagging: is it time to start lifting treatments? 4 signs

Dr. Park Hyungkwon2026.02.04

“Is it too early for lifting?” “People around me are already doing it.” “Won’t it be too late if I wait?”

These are common concerns among people in their late thirties to early forties.

Honestly, earlier is not always better, and neither is later. Treating someone too early may produce little noticeable change and disappointment; once sagging is advanced, devices alone may be insufficient.

Some days, I tell people who came for lifting that it is still too early. There is no reason to recommend it to someone who does not need it that year.

4 signals from your skin

Here are four signs you can check yourself. First, the jawline boundary: if the line between jaw and neck is less defined, sagging may have begun. Second, shadows around the nasolabial folds: deeper-looking shadows under the same lighting may suggest movement of cheek tissue. Third, recovery of elasticity: if the skin returns more slowly after gently pressing and releasing the cheek, dermal collagen changes should be considered. Fourth, photographs and makeup: compare a front-facing photo from a few years ago with today under the same lighting, and note whether makeup has begun collecting particularly in the nasolabial area. Photos often reveal changes missed in the everyday mirror. Matching the lighting matters, because morning daylight and evening indoor light cast different shadows on the same face. If two or three signs overlap, it may be worth having the condition assessed, regardless of whether you proceed with treatment. Self-checks are only a reference, however. It is difficult to tell whether visible changes come from the dermis or the fascia beneath, and different layers require different approaches. Recognizing signs and deciding on treatment are separate steps.

Prevention and correction require different plans

Lifting can be divided broadly into two approaches: preventive care for dermal elasticity before substantial sagging, and corrective care for changes already present. Some ask, “Is preventive lifting too early an investment?” The answer depends on the condition. At the preventive stage, we consider relatively less burdensome procedures to encourage dermal collagen regeneration and maintain the state of the skin. Devices targeting the dermis, such as Sofwave, are often considered here. Correction requires more detailed selection of devices and methods according to sagging depth and location. Devices such as Ultherapy, which address the area near the SMAS, enter the discussion, and when needed we consider combinations that assign different roles to different devices. Intervals also differ: prevention may mean widely spaced maintenance, while correction needs a focused plan tailored to the condition. The starting stage changes the design itself. Still, there is no reason to rush into treatment under the name of prevention when the skin shows no signs.

What we explain at LUNN

I often say during consultation, “You do not need to do it yet.” If condition is the criterion, there are times when no treatment is appropriate. If it is worth considering, I explain why and encourage you to decide after enough thought. LUNN has Ultherapy, Thermage FLX, Sofwave, and ONDA, allowing discussion of a suitable plan at either preventive or corrective stages. I personally handle consultation, treatment, and follow-up. We take enough time in a private room to show and explain the condition, so there is no need to rush a decision. I also explain that any procedure may cause temporary swelling or redness, and the degree and timing of results vary between individuals.

If facial sagging is beginning to concern you, first check which stage you are at before choosing a procedure. A place willing to say it is still too early is worth trusting.
