In accordance with the Medical Service Act, we disclose our non-covered treatment costs and certificate fees. Listed amounts include VAT and may change depending on circumstances.

Proxy Prescription
A proxy prescription is possible only when the patient is unconscious, or has serious difficulty moving and receives the same prescription for the same condition over a long period. A legal proxy recipient (lineal ascendant/descendant, spouse, or spouse's lineal ascendant) must present a family relationship certificate and the recipient's ID.
Documents Required for a Proxy Prescription
- 1.The proxy recipient's ID (copy accepted)
- 2.Family relationship certificate or resident registration copy
- 3.Proxy prescription confirmation (Ministry of Health and Welfare form, signed by patient or proxy)
