Sofwave delivers ultrasound uniformly to the mid-dermis in parallel beams rather than focusing it at one point.
It is reported to stimulate collagen and elastin regeneration, helping improve skin texture and fine lines.
However, evidence still comes from small studies, and effects and duration vary between individuals.
➊How does Sofwave work?
Sofwave is a noninvasive lifting procedure that distributes ultrasound energy evenly across the mid-dermis in broad parallel beams rather than concentrating it at one point.
Conventional ultrasound lifting focuses energy at points to reach deeper layers, but Sofwave uses a different method.
Multiple transducers emit parallel ultrasound simultaneously, delivering broad, even thermal stimulation throughout the mid-dermis at approximately 1.5mm.
This thermal stimulation partially denatures dermal collagen, which is understood to trigger the formation of new collagen and elastin.
Animal experiments observed dermal temperatures of 60–70°C immediately after treatment while surface temperatures stayed below 40°C, illustrating a design that selectively stimulates the dermis while protecting the epidermis.
Human biopsies also showed increased mid-dermal collagen-fiber density and longer, straighter rearrangement of elastic fibers 2 months after treatment.
➋How do Sofwave and Ultherapy differ?
Both use ultrasound for lifting, but energy delivery and target layers differ.
Focused ultrasound such as Ultherapy concentrates energy at points as deep as the SMAS (fascial layer), with depths including 1.5mm, 3.0mm and 4.5mm.
Sofwave instead spreads nonfocused parallel beams broadly at a single mid-dermal depth.
| Category | Sofwave | Ultherapy (focused) |
|---|---|---|
| Energy delivery | Nonfocused parallel beams | Focused point delivery |
| Target depth | Fixed at about 1.5mm in the mid-dermis | Variable 1.5–4.5mm, reaching SMAS |
| Main concerns addressed | Texture, fine lines, mild laxity | Structural sagging, fascial lifting |
It is more accurate to see them as tools for different layers rather than rank one above the other.
A comparative study of inner-arm laxity reported similar efficacy and no major pain difference between parallel and focused beams, with a shorter treatment time for parallel beams.
“Sofwave treats the mid-dermis, while Ultherapy treats deeper layers down to SMAS: they work at different levels.”
➌How do we decide whether Sofwave is right for me?
It may be especially helpful for mild to moderate laxity in people seeking gradual tightening and texture improvement without surgery.
Some research reports relatively better responses in early stages when nasolabial or marionette-line depressions are not yet deep.
With advanced sagging or substantial drooping of subcutaneous fat, ultrasound lifting generally may have limited effects, so combination with treatments targeting other layers is considered.
Because this energy is not affected by melanin, applicability is relatively broad even for darker skin tones.
- ✓Mild to moderate laxity: focus on Sofwave
- ✓Deep sagging or fascial issues: consider combining Ultherapy or other treatments
- ✓Both needed: choose by layer according to the objective

➍When do effects appear, and how long do they last?
One study found significant reductions in nasolabial and marionette-line depressions from around 1 month; over 60% of participants reported improvement then.
Further changes between months 1 and 3 were small, suggesting much of the change occurred early.
A pilot acne-scar study reported roughly 23–27% less depressed volume after just 1 session, with a mean 3.6-month follow-up.
However, studies to date are small and short-term, so evidence for long-term durability remains insufficient.
The degree and duration of firmness vary considerably with individual characteristics and care.
➎When it is suitable and when it is not
I recommend Sofwave to people who are not ready for surgical lifting but want natural improvements in texture and fine lines.
Downtime is relatively short and change may follow a single session, making it an option for those wanting minimal disruption to daily life.
With advanced sagging, Sofwave alone may not provide satisfying change, so I openly discuss combinations or alternatives.
As an emerging technology, it still lacks large, long-term studies; I explain this in advance and prioritize suitability over sales.
We assess laxity, skin thickness and expectations together to decide the plan (individual results vary).
“Sofwave suits mild to moderate laxity; advanced deep sagging calls for other approaches as well.”

QFrequently asked questions
Reported mean pain scores vary by study from 4 to 7 out of 10.
Air cooling is usually used alongside treatment to reduce discomfort.
Temporary erythema and swelling are most commonly reported and generally tend to settle within a day.
Because evidence is based on small studies, rare adverse effects cannot be ruled out.
Many studies reported significant improvement after just 1 session.
Additional treatment depends on individual laxity and goals.
They are best understood as tools for different layers rather than ranked as better or worse.
| Comparison | Details |
|---|---|
| Sofwave | Mid-dermis; texture and fine lines |
| Ultherapy | Down to SMAS; structural lifting |
We first assess skin condition and laxity, discuss a plan suited to your goals, then proceed to treatment.
The final plan is individualized through consultation (results vary).

Sofwave does not fill the skin; it helps the dermis make collagen again on its own.
Parallel-beam ultrasound stimulates collagen and elastin regeneration in the mid-dermis, helping improve texture and elasticity.
