The early stage, when the cheeks begin to sag and loss of firmness becomes noticeable, is often considered an appropriate time to consider facial lifting.
Early intervention, rather than waiting for sagging to progress, is reported to be favorable for recovery and maintenance.
However, skin condition and the pace of aging vary, so a clinical assessment is necessary.
➊When should you begin considering facial lifting?
Facial lifting is generally considered appropriate in the early stages of declining firmness, rather than only after sagging has visibly progressed.
Heavier-looking cheeks, a less defined jawline, or gradually deepening nasolabial folds may be signs.
These changes develop as collagen and elastic fibers in the relatively superficial skin layer, the dermis, gradually decrease.
The rate of progression depends on sun exposure, lifestyle, and genetics, rather than age alone, so it is difficult to set a fixed age threshold.
Among the patients I see, some already show signs in their early 30s, while others maintain relatively firm skin into their mid-40s.
Addressing the issue early, before sagging progresses, is generally considered favorable for recovery and maintenance.
➋Different lifting methods address different layers
Ultherapy PRIME delivers energy to a deep layer beneath the skin—the same layer lifted in surgical facial lifting.
That stimulation initiates a process in which the tissue gradually develops its own firmness.
If sagging itself is the main concern, addressing this layer may help.

Thermage uses heat to firm the relatively superficial dermal layer through tightening, while ONDA aims to improve both contour definition and firmness with relatively less discomfort.
Because these three procedures differ in the layers and approaches they use, the combination may vary with the degree of sagging and the desired result.
| Treatment | Main layer addressed | Suitable concern |
|---|---|---|
| Ultherapy PRIME | Deep layer beneath the skin | Noticeable sagging |
| Thermage | Relatively superficial skin layer (dermis) | Firmness and tightening |
| ONDA | Superficial to intermediate layers | Contour definition with less discomfort |
“Although sagging and loss of firmness may seem like the same concern, they can involve different layers and therefore require different approaches.”
➌How do you decide when facial lifting is right for you?
Timing is assessed by looking at current skin firmness, the extent of sagging, and lifestyle together—not simply age.
If your jawline looks less defined in the mirror or your face looks heavier in photos than before, it may be time for an assessment.

Even if sagging is not yet obvious, a preventive approach may be considered if firmness seems to be declining rapidly.
Conversely, if sagging has already progressed considerably, a plan combining several methods often helps improve satisfaction compared with a single procedure.
- ✓Early sagging: Focus on procedures that improve firmness
- ✓Progressive sagging: Consider both deeper-layer treatment and a combined plan
- ✓Prevention: Combine lifestyle management with regular assessments
➍When do facial lifting effects become noticeable?
Facial lifting often produces gradual rather than immediate changes.
Because collagen regeneration follows the procedure, noticeable changes are typically reported to begin around four weeks and progress toward three months.
| Time | Progress |
|---|---|
| 1–2 weeks | Recovery from temporary reactions such as swelling and redness |
| Around 4 weeks | Changes in firmness begin to be noticeable |
| 2–3 months | Continued improvement as collagen regenerates |
This course may also vary with the treated area and method, so it is best to obtain specific guidance during your consultation.
➎Can lifting procedures have side effects?
Temporary swelling, redness, and pain may occur after lifting, and rare adverse effects such as nerve injury or burns have also been reported.
This is one reason we carefully review your current skin condition and medical conditions during consultation.
I recommend postponing treatment during pregnancy or when there is acute facial inflammation; this too must be assessed individually in consultation.
Please bear in mind that both results and adverse effects vary between individuals.
➏Who I recommend lifting for, and who may need another approach
I recommend facial lifting to people whose firmness has started to decline but whose sagging is not severe, or to those with progressing sagging who prefer a nonsurgical approach first.
In these cases, earlier intervention tends to be associated with greater satisfaction.
However, if sagging has progressed substantially and there is considerable excess skin, lifting procedures alone may have limitations, so I recommend considering other options as well.
Pushing treatment at that stage might help the clinic's revenue, but it could reduce satisfaction with the result. That is why I do not recommend it indiscriminately.
We assess both the degree of sagging and the skin's condition to decide the direction of treatment in consultation (individual results vary).
“Lifting is not a universal solution for every degree of sagging. It is meaningful when tailored to the person's condition.”
QFrequently asked questions
Yes. Starting when firmness first begins to decline, before sagging becomes prominent, is recommended from a preventive-care perspective.
However, whether it is necessary should be decided after assessing the skin.
Treatment can still be performed after sagging has progressed substantially, but reports suggest satisfaction and recovery speed may differ from earlier intervention.
Individual differences make it difficult to generalize.
The pace of declining firmness varies, and early signs can appear in the 20s.
However, most people tend to consider it from their mid-to-late 30s onward.
If sagging is the main concern, a procedure addressing deeper layers may be considered. If firmness and texture are the goals, a different combination may be appropriate.
The best decision is made together after assessing your condition in consultation.
| Concern | Approach to consider |
|---|---|
| Mainly sagging | Treatment addressing deeper layers |
| Mainly firmness and texture | A combination of superficial-layer tightening methods |
Duration varies with the treatment method and depends greatly on skin aging and ongoing care.
You can receive specific guidance during consultation.
We assess skin firmness and the degree of sagging, then use those findings to design the treatment approach before proceeding.
A consultation is needed to identify the most appropriate approach for you (individual results vary).
Facial lifting is presented here as a procedure that helps bring forward the skin's recovery of firmness, rather than artificially changing the face.
The early signs of declining firmness, before sagging progresses, are often considered an appropriate time to consider facial lifting.
