Eye Thermage for fine lines: how does it differ from Thermage?

Eye Thermage for fine lines: how does it differ from Thermage?

Eye Thermage is a radiofrequency treatment using a small tip designed for the eye area, allowing more precise treatment of fine lines around and under the eyes.

While standard Thermage treats broader areas of the face, Eye Thermage adjusts the tip size and energy delivery for the thin, sensitive tissues around the eyes.

Both treatments, however, produce gradual effects, and individual results vary.

➊What is Eye Thermage?

Eye Thermage is a nonsurgical tightening treatment that delivers radiofrequency (RF) energy through a small eye-area tip to encourage collagen contraction and regeneration in the surrounding tissues.

Thermage was originally developed to treat the whole face using monopolar radiofrequency. Eye Thermage is the version that uses a separate small tip for thin, sensitive areas around the eyes.

Radiofrequency generates heat through the electrical resistance of the tissue itself. This heat is understood to stimulate dermal collagen, producing both immediate contraction and subsequent regeneration.

The skin around the eyes is among the thinnest on the face and has little underlying fat.

Instead of a full-face tip, Eye Thermage therefore uses a smaller eye-area tip that allows precise energy control.

Treatment is often planned separately for the upper eyelids, lower eyelids, and outer corners of the eyes.

➋What differs between standard Thermage and Eye Thermage?

The key differences are tip size and energy planning, reflecting the different treatment areas and goals.

Standard Thermage uses a larger tip for broad areas such as sagging cheeks, nasolabial folds, and the jawline. Eye Thermage uses a smaller tip and more finely adjusted energy settings for the narrow, thin-skinned eye area.

The underlying principle is the same.

It is a two-stage response: heat causes collagen fibers to contract immediately, and new collagen is produced during recovery.

Because the eye area is more sensitive to burns and irritation, epidermal cooling and energy control need to be more precise, which is why a dedicated Eye Thermage protocol is used.

“Even with the same radiofrequency principle, thin tissues around the eyes require a dedicated protocol with a different tip size and energy plan.”

➌Which is more suitable for me: Eye Thermage or standard Thermage?

Eye Thermage treatment

The choice depends on the location and severity of your concerns.

Eye Thermage alone may be sufficient if your concern is limited to fine lines or mild laxity around the eyes. If you also have broader facial sagging, such as sagging cheeks or nasolabial folds, standard Thermage is generally considered first.

If both areas are affected, the treatments may be combined, depending on the clinical assessment.

Another consideration is the sensitivity of the area.

The thin skin and limited fat around the eyes may respond to heat and recover differently. Rather than applying facial settings unchanged, using a dedicated protocol is a reasonable approach to both safety and effectiveness.

Eye Thermage: key points
  • ✓If only the eye area concerns you: consider Eye Thermage first
  • ✓If overall facial sagging is the concern: consider standard Thermage first
  • ✓If both concern you: discuss whether to combine them in consultation

➍When will changes appear, and how noticeable will they be?

Some effects may be apparent immediately, followed by gradual changes over weeks to months.

This reflects the different timing of immediate collagen contraction and subsequent new collagen formation.

TimingTypical course
Immediately after treatmentSome improvement in firmness from immediate collagen contraction
After several weeksGradual improvement as new collagen forms
After several monthsCumulative improvement in wrinkles and firmness; individual results vary

The timing and extent of changes depend on skin condition, age, and the degree of laxity.

Some studies have reported greater satisfaction in people with more noticeable baseline sagging, so it is important to remember that individual starting conditions influence the outcome.

➎Side effects and situations requiring caution

The most common responses are redness and swelling immediately after treatment, which usually settle within a short time.

Temporary dryness or mild bruising may occasionally occur, but most cases resolve naturally.

Because radiofrequency uses heat, inappropriate energy settings can cause burns, pigmentation changes, or, rarely, hollowing from damage to the fat layer.

Epidermal cooling, real-time temperature control, and energy settings tailored to the patient are therefore important.

If your face is lean or the fat layer around your eyes is thin, energy settings and treatment areas require particular care.

Treatment may be restricted if you have an implanted electronic device such as a pacemaker, metal or silicone implants in the treatment area, an active infection, or are pregnant.

➏Who may or may not be a suitable candidate?

I tend to recommend considering Eye Thermage when fine lines or mild laxity around the eyes have just become a concern, but surgical correction is not yet being considered.

Older patients may have more pronounced baseline laxity, and related studies have reported relatively greater perceived satisfaction in such groups.

Conversely, if eyelid drooping is pronounced enough to affect vision, radiofrequency alone has limitations, and surgery such as blepharoplasty may be more appropriate.

If dark circles or protruding under-eye fat are the main concerns, another approach may be needed. The priority is to explain when this treatment cannot address the concern.

The degree of laxity and your desired outcome are assessed together in consultation to decide whether to proceed; individual circumstances vary.

“It may be considered when lines around the eyes are beginning to bother you, but if sagging is pronounced, surgery may offer a more fundamental correction.”

QFrequently asked questions

How many Eye Thermage sessions are needed?

It is often performed as a single session. The appropriate number is determined after assessing the eye area in consultation.

Should I have standard Thermage or Eye Thermage first?

Standard Thermage is considered first for general facial sagging, while Eye Thermage is considered first when fine lines around the eyes are the main concern.

They may also be performed together if both areas are a concern.

Is Eye Thermage relatively comfortable?

The small eye-area tip includes cooling, so treatment is often performed without anesthesia. However, discomfort varies from person to person.

Does it also improve dark circles or under-eye bags?

Eye Thermage focuses on wrinkles and firmness; it is not a treatment that directly resolves dark circles or protruding under-eye fat.

How is Eye Thermage performed?

After examining the eye area, the clinician plans the tip size and energy settings and performs the treatment. Individual needs vary.

Rather than filling or mechanically pulling the eye area, this treatment signals the tissues to produce collagen again.

Eye Thermage is a nonsurgical treatment that precisely delivers radiofrequency energy through an eye-area tip to improve fine lines and laxity.
