Adult acne at work: is Gold PTT the answer? 3 honest answers

Dr. Park Hyungkwon2026.04.01

"It keeps appearing only on my chin." "Once I stop going to the dermatologist, it comes back." "Would it be better to start with a lifting treatment?"

These are things I hear from working adults seeking help for acne.

To be frank, adult acne is not a condition treated once and finished. And if you start with lifting while acne is still breaking out, the results of both are usually disappointing.

In this situation, I do not recommend lifting first. Changing the order can change the outcome even with the same budget.

Why does adult acne recur in the same places?

When asked, "Will washing more thoroughly help?", I unfortunately have to explain that this alone is unlikely to be enough. Adult acne often develops as stress, insufficient sleep and hormonal changes combine to overwork the sebaceous glands. Unlike adolescent acne, which tends to spread over the forehead and nose, adult acne often develops as deep, painful lesions around the chin, mouth and jawline. New lesions arise beside older ones before they settle, and lesions may persist without coming to a head even when pressed. Because the central problem lies in the sebaceous glands rather than just the skin surface, changing cleansers or cosmetics may help temporarily, only for acne to return in the same spots when your general condition deteriorates. This is why calming visible inflammation alone can leave you starting again during the next week of overtime. If you ask, "Does that mean it's just my constitution and nothing can be done?", my answer is no. It simply means that if the source is the sebaceous glands, treatment should also target them. Once we decide where to interrupt the recurrence cycle, the treatment becomes much clearer.

Why treat acne before lifting?

Some people ask, "Sagging feels more urgent. Can't we do both?" The short answer is that keeping the right order is ultimately faster when inflammation is active. Repeated inflammation leaves red marks, pigmentation and sometimes scars. Even if lifting refines the contours, remaining acne marks can limit the change in the skin's overall impression. Focusing only on lifting while inflammation is present is like wallpapering a house before fixing its leaking roof. Calming inflammation first gives later lifting or skin-booster treatment a better chance to work as intended and reduces the accumulation of additional issues such as marks and pigmentation. Reversing the sequence can leave both treatments disappointing. Once acne has stabilized, the starting point for subsequent procedures is different. Red marks and pigmentation can then be addressed gradually with lasers or skin boosters, and lifting consultations can also become more precise. A path that respects the sequence often takes less total time than the one that initially looks quicker.

What does Gold PTT target?

Gold PTT delivers tiny gold particles through the pores to the area around sebaceous glands, then uses a laser to selectively heat the area and regulate overactive glands. Because it targets a central contributor to acne, improvement in recurring adult acne may be expected. Unlike an approach that only dries out surface inflammation, it addresses the starting point of recurrence. This is also why I explain it early to working adults who feel burdened by the question, "Do I have to keep taking antibiotics?" Treatment generally requires several sessions at intervals of 2–4 weeks, and temporary redness or stinging can occur afterward. Many people can return to daily activities on the day of treatment, so the scheduling burden is often relatively small. Results vary, and the number and spacing of sessions are adjusted to sebum production and skin condition. I believe it is more honest to explain in advance that this is not a one-session treatment. Depending on inflammation severity, we may also combine extraction, anti-inflammatory injections or medication when needed. This is not because Gold PTT is the only answer, but because we plan separately for the sebaceous glands driving recurrence and the inflammation already present.

How we plan the sequence

At LUNN, I personally diagnose the skin condition and plan the order of Gold PTT and subsequent care. Once breakouts stabilize, we gradually address remaining red marks or pigmentation, then revisit lifting. With one doctor continuing from diagnosis through treatment and follow-up, the plan can be adjusted to the skin's response at each session. Many patients come between busy commitments, so we agree on session intervals around their work schedules. We are a one-minute walk from Exit 4 of Apgujeong Rodeo Station, and some working patients visit on their way home. Consultations take place in private rooms so patients can discuss their acne comfortably. Of course, procedures are not everything. I always explain that sleep and stress management need to accompany treatment to extend the intervals between recurrences. Even if overtime cannot be eliminated, establishing a skincare routine for the following day may help limit flare-ups. I consider finding that practical compromise together part of the treatment plan.

If chin acne keeps recurring, I recommend first identifying why it recurs rather than choosing a procedure. Any practice that explains that can be a good place to seek care.
